A leading cause of forklift fatality and serious injury accidents is due to mechanical failure. It has been proven that properly inspecting the forklift before use reduces the number of accidents. The forklift's operator's or safety manual should detail the proper inspection process for your particular machine. The machine must be taken out of service, if the forklift operator finds a defect during the inspection process.
29 CFR 1910.178 (p) (1): “If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition.”
29 CFR 1910.178 (q) (7): “Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily.” “Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected.”
OSHA does not require a written record of forklift inspections. However, there are good reasons for documenting these examinations.
· Completing a checklist helps to ensure that items are not missed during the process.
· A completed forklift inspection checklist allows operators to report defects.
· Keeping a written record of the forklift inspection can be used as evidence that the machine was examined.
· When problems are found during the inspection, noting these issues alerts maintenance personnel.
OHSA has defined seven classes of powered industrial trucks with several subclassifications. If you are unsure of which forklift you have, you can contact the forklift’s manufacturer. Oddly enough, most forklift owner’s manuals do not provide this information. You can also visit an OSHA website that details each class and subclassification.
These powered industrial truck inspection forms allow forklift operators to document the daily or pre-shift inspection. The documents can be printed on both sides of the page, and the margins are indented for use in a three-ring binder. There is also a text box for your company name.

If the checklist for your forklift is not available, simply find the document that is closest to your forklift, and make any necessary changes.
